GENERAL INFORMATION

FEE: $30 per semester

AFTERNOON HOURS: 1:00 - 2:30 PM

SESSIONS: October - December / January - May

SNACK: Each student must bring two snacks (to feed 20 students) and two bottles of juice (per semester)

IMMUNIZATION SHEET: Must be filled out completely and signed by a doctor

INFORMATION CARD: Must be completely filled out in case of any emergencies (Click here to view a printable copy)

CHANGE OF CLOTHES: Each student must bring one change of clothes (in case of any accidents)

SUPPLIES: Must be labeled with child's name
