Leyden High Schools: Enrollment Information

 In order to enroll a new student, you will need to complete an enrollment packet available from the Assistant Principal’s Office. Please bring the following information and documentation:

1. PROOF OF RESIDENCE consisting of either a real estate tax bill, a lease or a closing statement.  In addition, two other bills must be brought in (gas, electric, water, etc.) and a picture ID of the parent/guardian (drivers license).

2. A BIRTH CERTIFICATE OR PASSPORT for the student.

3. AN ILLINOIS SCHOOL PHYSICAL EXAMINATION and a record of immunization for every entering student.

4. A COPY OF TRANSCRIPTS and the name and address of the last school the student attended will help facilitate the enrollment process.  Incoming Freshmen should bring in their last report card or diploma.

5. A COPY OF EXPLORE TEST RESULTS, IF AVAILABLE. If the student has not taken the EXPLORE test,  a Leyden Guidance Counselor will schedule a time to take this test to ensure correct course placement.

OTHER MATERIALS:

6. A COPY OF IEP for special education students only.

7. ILLINOIS STATE BOARD OF EDUCATION STUDENT TRANSFER FORM, if the student is transferring from an Illinois Public School. Out-of-state transfers must submit a form/affidavit stating the student is not currently serving a suspension or expulsion imposed by the school from which the student is transferring.

PLEASE NOTE: The enrollment process also minimally includes the following; a residency check, an interview with an administrator, a placement test for 9th and 10th graders, and course selection with a counselor.

FEES:

o The District charges a $100.00 registration fee due on the student's first day of school. Application for waiver of fees is available upon request.

o Students are responsible for purchasing a physical education shirt and shorts at the bookstore.  Size L, or XL shirt and shorts (set) cost $16.00; XXL costs $17.00; XXXL costs$18.00.

o Students are responsible for purchasing a graphing calculator for all mathematics classes. Additional information available from student’s mathematics teachers.
