The following are some resources to aid you in the college application process. If you have any questions, please see your counselor for more information.

 
         
  FUTURE PLANNING WORKSHEET - This form can be filled out and returned to your counselor. It helps you to consider what types of colleges you are considering attending and helps your counselor to better assist you in the college search process.      
         
  RECOMMENDATION FORM - If you are applying to colleges or scholarships that require letters of recommendation, fill out this form and give it to those people whom you have asked to write recommendations (teachers, counselors, coaches, etc.). It helps those people learn more about you so they can write the recommendation for YOU. Remember to give people at least two weeks prior to the application deadline to write the letter.      
         
  TRANSCRIPT REQUEST FORM - You need to submit an OFFICIAL transcript to each college to which you are applying. Use this link to download a transcript request form. All forms should be returned to the Registrar.
