FRESHMEN MEDICAL FORM INFORMATION

June 1, 2012 - All freshmen medical forms are due in the Health Office.

Your student cannot start school in August without having a freshman physical dated within one year of entering high school and also having all the required immunizations.

IMPORTANT: Please be sure that all required components of the form are completed. This includes the dates of all the immunizations (these can be obtained from your physician or from your child's grade school) and the health history portion (located on the back of the form) completed and signed by you. The physical examination must have all of the requirements completed, signed and dated by the physician with his/her name and address. This will avoid us having to return the form to you for completion.

After the June 1st deadline, a sliding scale of late fees apply for freshmen medical forms received on the following dates:

June 2 - June 30: $30.00 late fee
July 1 - July 31: $40.00 late fee
After August 1st - $50.00 late fee

All late fees will be due no later than Thursday, August 9, 2012.


 

The All Kids program offers Illinois’ uninsured children comprehensive health care that includes doctor’s visits, hospital stays, prescription drugs, vision care, dental care and medical devices like eyeglasses and asthma inhalers. Parents pay monthly premiums for the coverage, but rates for middle-income families are significantly lower than they are on the private market. For instance, a family of four that earns between $42,000 and $63,000 a year pays a $40 monthly premium per child, and a $10 co-pay per physician visit. Illinois Health Connect

Illinois Health Connect is a new health care program of HFS (Illinois Department of Healthcare and Family Services). Illinois Health Connect helps you and your family to get health care and stay healthy by providing a medical home. Most people with an HFS or All Kids medical card MUST pick a medical home.
