Student Visit Policy

  • All students who come to the office for an injury/illness must have a pass from the class they are missing- with the exception of an emergency. No passes are needed during a medical emergency. Students are not allowed to come in during the passing period. The only exception is during their lunch period. All students must sign in the Student Log upon entering the health office and sign out when leaving.

    Failure to sign in and/or out can result in their time in the health office not being covered for attendance. The nurse will assess the student to determine the appropriate action to be taken. If the student is being released to go home, we must speak with a parent/guardian before the student is released.

  • **NEW - Class of 2024 Illinois Immunization Requirement 12th Grade Meningitis Booster (Includes Transition & 5th year Seniors)

    Please be aware that the State of Illinois has added NEW immunization requirements. Any student entering the 12th grade shall show proof of receiving 2 doses of Meningococcal immunization (MCV4, Menveo, Menquadfi, Menactra). Minimum intervals for administration: The first dose is received on or after the 11th birthday; the second dose on or after the 16th birthday. The required interval must be at least eight weeks after the first dose. Only one dose is required if the first dose was received at 16 years of age or older. No proof of immunity is allowed.

    Students that are not compliant by October 15, 2023 are subject to exclusion until the appropriate documentation has been submitted according to the ILDPH code.

    2023-2024 Immunization Requirements 

     

    **NEW - CLASS OF 2026 CHILD DENTAL EXAM REQUIREMENT

    As of the 2019-20 school year, Illinois school code now requires students entering 9th grade to have a dental examination before May 15th of their freshman school year. Freshman students need to submit proof of a dental exam (or waiver) by May 15, 2023. School dental examinations must have been completed within 18 months prior to the May 15, 2023 deadline. If a student fails to provide documentation by May 15, 2023, the student's report card may be held.  State of IL Code (HP 4908/PA 100-0829 Section 27-8.1)
