Community Service Hours

Once you have completed service hours towards your Committed to Excellence Project, you must enter them below. Be sure to fill in the form completely and accurately to ensure your hours are recorded properly. Also, be sure to include the contact information of the person you reported to for the project. Remember, you must submit your hours for EACH PROJECT COMPLETED.
